As such embassy doesn’t ask for any documents, but it will be safe to carry the below documents in case they want to cross check

Valid Passport with at least 6 Months validity from the date of travel

1 Visa Form … We will fill up based on info. provided to us

Covering letter on letter head [with traveller names, Passport number, travel date, purpose of travel, designation on signatory and co. stamp) Covering should clearly state : Expenses clause : All expenses including travel, accommodation and local expenses while in USA will be completely bourne by myself]

Accommodation proof :  hotel booking / Invitation from relative – Passport copy, address, Tel. no. of local contact

Personal Income Tax Returns of last 3 years

Personal Bank Statement of last 6 months

Salary Slip for last 3 months if working

Investment documents of candidates

US VISA renewal … For VISAs issued after 1^st Jan. 2009

Current & old passport if the US VISA is in old passport

Photocopy of 1^st page, last page & USA VISA page

Roughly filled up attached VISA form for us to fill up online … 1 form per person is required   

2 photographs : 2x2 inches, white back ground, face should be 70% of the photograph, shoulders visible, no specks, no smile, straight head 

Once the VISA form is uploaded, dates for submission of passport is selected 

We will submit and collect the passport on your behalf
